September 23, 2016 By WRFA Radio Leave a Comment

Chautauqua County SheriffHARMONY – Chautauqua County Sheriff Deputies are continuing an investigation into a fatal crash that took place late Thursday afternoon involving a vehicle and an Amish buggy.

The sheriff’s office says the crash took place just before 5 p.m. in the town of Harmony. According to the police report, Peggy Miles of Panama – whose age was not released – was traveling on Weeks Rd in the Town of Harmony and reportedly fell asleep. Miles continued on and ran into the back of an Amish Buggy, which had multiple occupants.

The operator of the Amish Buggy, identified as Henry Koffman Jr, was pronounced dead at the scene scene. Other passengers included Rachel Koffman along with three children. Rachell Koffman and one child was flown to UPMC Hamot for their injuries via Star Flight. The two other children were transported to UPMC Hamot via ambulance.

The Chautauqua County Accident Reconstruction team was called out to process the scene. The investigation is ongoing.

The Chautauqua County Sheriff’s Office is also once again reminding motorists to use extra caution when sharing the roadway with Amish Buggys.
